All PADI certification require that a medical questionnaire be completed and approved prior to participating in the confined or open water portion of the class.  If you plan on starting a course, get a head start on the paper work by completing your Medical statement ahead of time.  Answer all the questions with a "Yes" or "No". If all your answers are "No" then you're good to go.  If you have any "Yes" answers then simply take the medical statement to your general practitioner to have them sign off on your suitability to dive. Once we have their signed approval we'll get you in the water and having fun!

Click the PADI Medical Statement link below to print off and complete at home or review to see if you'll need a doctor's approval.
